NEW CLIENT Intake Inquiries
We are currently able to accommodate a limited number of new clients, and will be working to ensure best match of new clients to members of our clinical team. If you are interested in becoming a client, please complete the form below and our intake counsellor will connect to follow-up.
* Please note that sessions with our practicum student are at a reduced rate.
At present, we are unable to guarantee we will have ability to meet all inquiries and requests; however, we do commit to returning messages and providing referral options, should we be unable to assist.

1) Your name
2) Contact information – phone number or email address
3) A brief summary (2-3 sentences) on what you are hoping to discuss with a counsellor
4) Name of preferred clinician – if you have one
